Catch health changes early
With charts, thresholds and nurse oversight on every shift.
Your workers record observations and charts on the shift, and anything outside a participant’s safe range reaches a nurse or team leader straight away. Vitals, bowel, fluid, seizure and sleep charts, mealtime management and high-intensity supports sit in one participant record.
Free for 14 days, with rostering, the support worker app, progress notes and NDIS invoicing. Set it up yourself online.
What changes for you
Out-of-range readings escalate on the spot
Thresholds set per participant trigger alerts to the nurse or team leader as soon as a reading or chart entry falls outside them.
High-intensity supports done to plan
PEG, catheter, wound, pressure and dysphagia supports follow the participant’s plan, with only trained workers recording them.
Trends ready for the GP and allied health
Charts build over weeks and months, so a GP, nurse or speech pathologist sees the pattern rather than a single reading.
From reading to response
- Step 1Plan and thresholds set
Nurse sets the charts and safe ranges for the participant.
- Step 2Recorded on shift
Workers chart observations and supports on the app.
- Step 3Alert raised
Out-of-range or overdue entries notify the nurse or team leader.
- Step 4Clinical review
The nurse reviews, records actions and updates the plan.

The right texture, every meal
For participants with dysphagia, the mealtime plan is on the worker’s phone before they serve: IDDSI food and drink levels, positioning, pace and the assistance required. Workers confirm the plan was followed and record any coughing, choking or refusal, which can raise an incident and alert the nurse.
- IDDSI food and drink levels
- Positioning and assistance shown before meals
- Choking or coughing recorded and escalated
- Speech pathologist plan attached

Explore Care plans and goals →Questions buyers ask
Can we set different thresholds for each participant?
Yes. Safe ranges are set per participant and per chart by a nurse or authorised clinician, and alerts go to the people you choose for that participant or house.
Do we need nurses to use the health module?
No. Workers can record charts and observations without a nurse on staff. Where you do have nurses, they get a review view across their participants and can manage plans and thresholds.
Can allied health see the charts?
You can share chart reports with GPs, speech pathologists and other allied health. Shared access for external clinicians on the participant record is part of the connected care network.
