Progress notes you can stand behind at audit
Written on shift, tied to goals and reviewed before they reach the record.
Every shift ends with a complete, readable note linked to the participant’s goals. Support workers fill a structured template for the service type on their phone, attach photos where needed, and team leaders review and sign off before the note becomes part of the participant record.
Free for 14 days, with rostering, the support worker app, progress notes and NDIS invoicing. Set it up yourself online.
What changes for you
No shift closes without a note
Notes are written against the shift itself, with mandatory fields per template, so missing or half-finished notes are flagged before timesheets and claims go out.
Evidence of progress against goals
Each note is tagged to the participant’s plan goals, so plan reviews and reports draw on months of real observations rather than a last-minute summary.
Notes that hold up to scrutiny
Time-stamped, attributed and locked after review, with every amendment kept in history, so the record stands in an audit, a complaint or a reportable incident investigation.

Team leaders see every note before it counts
Submitted notes land in a review queue by house, team or participant. Team leaders read, approve or send back with a comment, and anything that describes harm, a near miss or a restrictive practice can be raised as an incident straight from the note, with the note attached.
- Review queue by house, team or participant
- Return to worker with comments
- Raise an incident from the note
- Amendments kept in history

Can we have different note templates for SIL and community access?
Yes. Templates are set per service type, each with its own sections and mandatory fields, and the right template opens automatically for the shift the worker is on.
Can a note be changed after it is submitted?
Before review, the worker can correct it. Once approved it is locked; any amendment is recorded as a new version with the author, time and reason, and the original stays in history.
Can participants or families read the notes?
You can share plain-English summaries with participants, nominees and guardians today. Direct access through the participant and family portal is coming soon.
Does Axis AI write notes for workers?
No. Axis AI is coming soon and is designed to check a worker’s own draft for missing detail and clarity, not to write the note for them. The worker remains the author.
