Incidents and complaints

Never miss a reportable incident deadline

From the worker’s first report to the corrective action that closes it.

Incidents are reported from the shift, triaged within minutes and classified against the NDIS reportable incident rules. Timers run for 24-hour and 5-business-day notifications, the Commission notification is prepared for lodging, and investigations and corrective actions are tracked to close.

What changes for you

Reportable deadlines in plain sight

The most serious incidents must be notified to the NDIS Commission within 24 hours and others within 5 business days. Timers start on report and escalate as they run down.

Every incident followed through

Investigation, corrective actions and participant follow-up are assigned with due dates, so incidents are closed properly rather than just logged.

Patterns seen before they repeat

Trends by participant, house, type and time of day show where risk is building, so you can act before the next incident.

Know within minutes whether it is reportable

When an incident comes in, the team leader answers guided questions about what happened and who was involved. If it is reportable, the right timeline is set, the countdown is visible to management, and the notification to the NDIS Commission is drafted from the record for your team to check and lodge.

Questions buyers ask

Does CompanionAxis lodge reportable incidents with the NDIS Commission?

It prepares the notification from the incident record and tracks the deadline. Your team reviews it and lodges it through the NDIS Commission’s portal, then records the lodgement in CompanionAxis.

Can workers report an incident if they are not sure it counts?

Yes. Workers report what happened and the team leader or quality team classifies it, including near misses. It is better to report and triage than to miss one.

Are complaints kept separately from incidents?

Complaints and feedback have their own register with acknowledgement and resolution tracking. A complaint can be linked to a related incident where both apply.
