Connected care network

Work with the participant’s whole team

Support coordinators, plan managers, allied health, behaviour practitioners, families and other providers see the parts of the record they need.

A participant’s supports rarely sit with one provider. CompanionAxis gives each member of their wider team limited, consented access to the goals, notes, incidents, budgets and reports relevant to them, takes referrals in from support coordinators and shares service agreements and reports without email chains.

The connected care networkYour organisation keeps one participant record at the centre. Support coordinators, plan managers, allied health professionals, family or nominees and other providers see what consent allows, and their referrals, reports and updates come back into the same record.Participant recordYour organisationCoordinatorReferral and planPlan managerInvoices, budgetsAllied healthGoals and reportsFamilyShared shift notesProvidersShared supports

What changes for you

Everyone works from the same picture

The support coordinator sees goal progress, the behaviour practitioner sees incidents and behaviour data, the plan manager sees what was delivered, so nobody chases updates by phone.

Shared only with consent

Each person gets access to the parts of the record the participant or their nominee has agreed to, and every view is written to the audit log.

Referrals to first shift, faster

Referrals from support coordinators arrive with the plan details and goals attached, so intake, the service agreement and the first roster happen in days, not weeks.

How the care network works

  1. Step 1Referral arrives

A support coordinator refers the participant with plan details and goals.

  2. Step 2Consent recorded

The participant or nominee agrees who can see which parts of the record.

  3. Step 3Access granted

Each team member gets a login limited to what consent allows.

  4. Step 4Work is shared

Service agreements, notes, incidents and reports reach the people who need them.

  5. Step 5Families follow alongSoon

The participant and family portal will show shifts, workers and shared updates.

See it in the app a closer look at the screens your team uses every day.

Plan reviews without the paper chase

When a participant’s plan is due for review, the support coordinator needs evidence of progress against goals. With access to the goals and progress notes the participant has consented to share, they can see that progress as it is recorded, and your team shares the plan review report from the record instead of compiling it by hand.

  • Goal progress visible as it happens
  • Reports built from recorded notes
  • Consent recorded for every person
