Build goal evidence from shift notes, record progress ratings, and close goals when they are achieved or no longer relevant.
Before you begin
- Goals added to the participant’s care plan (see Building a care plan and linking it to NDIS goals).
- Record goal progress to rate goals; Manage goals to mark them achieved or discontinued.
Tag goals in shift notes (support workers)
- In the support worker app, write the progress note at the end of the shift.
- Under Goals this note supports, tap each goal the shift worked towards.
- Use the Goal progress prompt to describe what happened, then save the note. Each tagged note counts as evidence for that goal.
Record a progress rating
- Open the care plan from People → Care plans and goals and go to Goals and outcomes, or select Record progress on the participant’s Goals and care plan tab.
- On the goal card, select Record progress.
- Enter Progress (%), choose a Rating from 1/5 to 5/5, and add a Note.
- Select Save rating. The progress bar, Latest rating and the Outcome trend chart update.
Check the evidence
- On the participant’s Goals and care plan tab, each goal shows how many notes are linked as evidence.
- Select See evidence notes to open the notes tagged to that goal.
- On the care plan, the Goal prompts in shift notes card shows when each goal was last rated.
Close or reopen a goal
- Select Mark achieved when the participant reaches the goal.
- Select Discontinue when the goal no longer applies.
- Select Reopen on a closed goal if it becomes relevant again.
Tips
- The rating scale runs from Needed full support (1) to Independent (5).
- When progress drops twice in a row the goal becomes Needs attention and the coordinator is notified.
- Progress notes without a goal tag can be flagged, so team leaders can coach workers during note review.
If something goes wrong
A worker sees “No goals recorded” in the note composer
The participant has no active goals, or the worker is offline (goals show when online). Add goals on the care plan.
Record progress isn’t shown on a goal
Only active goals (not started, on track, needs attention) can be rated, and you need Record goal progress.
Save rating stays greyed out
Enter a progress percentage or choose a rating; at least one is needed.
