Risk assessments and when to review them

Complete participant risk assessments, keep them current with schedules and review dates, and connect them to the participant’s risk flags.

Before you begin

  • Access to Quality → Assessments and permission to start or assign assessments.
  • The participant record exists.

Start or assign an assessment

  1. Go to Quality → Assessments, or open the participant’s Documents tab and select Assessments in the Assessments card.
  2. Select New assessment, or use its arrow to choose a type, for example Participant risk assessment, Falls risk screening or Pressure injury and skin integrity.
  3. Choose the participant as the subject.
  4. Select Start now to fill it in yourself, or Assign to give it to someone with a due date.

Complete the assessment

  1. Work through the pages. Continue and Save draft keep your answers.
  2. Rate each risk on the likelihood and consequence matrix and record the controls.
  3. Submit on the last page. Assessments that need sign-off wait for it before they are complete.
  4. When the participant’s risk assessment is complete, the Risk assessment completed intake checklist item ticks itself.

Know when each assessment is due again

Each type has a default validity in the starter pack, for example 12 months for the participant risk assessment and home and environment safety, 6 months for falls risk screening and 3 months for pressure injury and skin integrity.

  1. Open the Due and overdue tab to see assessments that are Expiring, Expired or Overdue.
  2. Select Remind to nudge whoever is assigned, or Re-assess to start the next one.
  3. Re-assess early when something changes, such as a fall, hospital stay or a new risk flag.

Schedule assessments automatically

  1. Open the Schedules tab and select New schedule.
  2. Choose the assessment, Applies to (for example Every active participant), the Frequency and Lead time, and who it is assigned to.
  3. Select Create schedule. Schedules assign assessments each night ahead of the due date, and new participants are picked up automatically.

Keep risk flags in step

  1. After the assessment, open the participant’s Risk flags card and add or update flags with a clear Alert workers see.
  2. Set Review on for each flag to match the assessment’s next review.

Tips

  • The participant’s Documents tab lists every assessment with its completed, due and expiry dates.
  • Use Pause on a schedule during a planned break in services rather than deleting it.

If something goes wrong

A participant can’t be made active because Risk assessment completed is open

Complete and submit the participant risk assessment (and any sign-off), or waive the item with a reason if your policy allows.

An assessment stays Awaiting sign-off

The type requires a sign-off. The person with the sign-off role must open it and sign.

You can’t find an assessment type

It may be disabled for new assessments in the types settings. Ask a quality administrator.
