Uploading documents to a participant record

Store plans, agreements, reports and other files on the participant record with the right sensitivity, expiry and review dates, and choose whether support workers can see them.

Before you begin

  • Upload participant documents plus file upload permission; Manage participant documents to delete.
  • The file ready on your computer.

Upload a document

  1. Open the participant and select the Documents tab.
  2. In Plans, assessments and reports, select Upload. The Upload a document window opens.
  3. Choose the File. The Title fills from the file name; change it if needed.
  4. Choose the Type: NDIS plan, Service agreement, Assessment, Medical, Consent, Report, Behaviour support or Other.
  5. Choose the Sensitivity: Personal, Health or Restricted.
  6. Set an Expiry and a Review date if the document runs out or needs checking.
  7. Tick Workers can see it in the worker app only if support workers need it on shift, then select Upload.

Find, download or remove documents

  1. Search the table or filter it by Type. The Expiry and Review dates turn amber within 30 days and red once passed.
  2. Select the download icon to open a file.
  3. Select the delete icon and confirm to remove a document uploaded in error.

Other places documents are added

  1. Service agreements attached during intake are kept on the record.
  2. Signed consent forms uploaded in the Consent register are kept as evidence.
  3. Assessments and service agreements have their own cards at the top of the Documents tab.

Tips

  • Reminders go to the participant’s coordinator before a document expires or is due for review (by default 30 and 7 days before).
  • Upload a guardianship order with its end date as the Expiry, and record the guardian’s authority end date on the contact too.
  • Choose Restricted for documents only a few people should open.

If something goes wrong

There is no Upload button

You need both permission to upload participant documents and to upload files. Ask an administrator.

A colleague can’t open a document you uploaded

Check its Sensitivity. Health and restricted documents need matching access. If it was set too high, delete and re-upload it with the right sensitivity.

Delete fails with a version or conflict error

Someone changed the document since you loaded the page. Refresh and try again.
