Reporting a medication error as an incident

Report a medication error, near miss or unexplained stock discrepancy as an incident, linked to the medication record.

Before you begin

  • Your role can create incidents.

Start the incident

  1. Go to Care → Medication and eMAR and select Medication incident at the top. The Log an incident wizard opens with the incident type set to the medication error type.
  2. From an exception, select Raise incident on it in Exceptions to escalate; the incident is linked to the exception.
  3. From the participant’s medication view, the button preselects the person as well.
  4. In the support worker app, use Incident from the shift quick actions.

Complete the wizard

  1. What happened: choose the Participant involved, House, Date and time, Incident type, Where exactly, a one-line Summary and What happened (wrong medicine, dose, person, time or route, and what you did).
  2. Tick the immediate actions taken, add Witnesses and files, set Near miss if nobody was harmed, and choose an initial severity.
  3. Injuries and first aid: record any harm and treatment.
  4. Reportable check: answer the questions. See Deciding whether an incident is reportable.
  5. Notify people, then Review and submit and select Submit incident.

Track medication incidents

  1. Open the Audits and incidents tab. Medication incidents lists errors and exceptions raised as incidents in the last 30 days with their status.
  2. Select one to open the incident, or All incidents for the full register.

Stock discrepancies

  1. When a stock count doesn’t match, the count shows Discrepancy and is flagged to the team leader. If it can’t be explained, raise a medication incident.

Tips

  • Report near misses too; they show patterns before harm happens.

If something goes wrong

Raise incident is missing on an exception.

It needs permission to escalate medication exceptions and to create incidents.

The incident type isn’t preselected.

Your organisation’s incident types may not include the medication error type. Choose the closest type manually.
