CompanionAxis does not have a separate ABC chart screen. This article shows where antecedent, behaviour and consequence information is captured today and how to collect ABC data with a form.
Before you begin
- The participant has a behaviour support plan (see Uploading a behaviour support plan and its review date).
- To build a form you need access to Quality → Forms.
When a restrictive practice was used
The restrictive practice use record follows the ABC pattern.
- Go to Care → Restrictive practices and select Record a use.
- Use Antecedents (what happened before) for the A.
- Use What happened and why the practice was used for the B.
- Use Outcome and Strategies tried first for the C and the response.
- Select Save use. See Recording use of a restrictive practice on shift.
When there was a behaviour of concern but no restrictive practice
- Log an incident from Quality → Incidents → Log incident, or from the worker app with Report an incident.
- Choose the incident type Behaviour of concern.
- In What happened, describe what happened before, the behaviour and what happened after, in order.
- Under Immediate actions taken, tick Followed the behaviour support plan if you did.
- Submit the incident. See Recording an incident: what happened, who was involved and immediate actions.
Collect routine ABC data with a form
- Go to Quality → Forms and select New form.
- Build a short form with fields for date and time, setting, antecedent, behaviour, consequence and strategies used. See Building a form with the form builder.
- Publish it so workers can fill it in on shift.
- Share the responses with the behaviour support practitioner for the plan review.
Functional behaviour assessment
- Go to Quality → Assessments and select New assessment.
- Choose Functional behaviour assessment. It covers behaviours of concern (frequency, intensity, likely function), setting events, triggers, a summary hypothesis and restrictive practices.
- Complete and sign it off, then attach it to the behaviour support plan review.
Tips
- Workers can tick Possible incident on a progress note in the app; the app then helps them write an incident report.
- Incident trends by type, house and time of day are on Insights → Incident trends, which helps spot patterns in behaviours of concern.
If something goes wrong
I can’t find an ABC chart option.
There isn’t one yet. Use the restrictive practice use record, an incident of type Behaviour of concern, or your own form as described above.
Behaviour of concern is not in the incident type list.
Your organisation may have switched that type off. Ask your administrator.
