Write the progress note for your shift in the app: choose the participant, tag their goals, record how they were, write what happened and flag anything your team leader needs to know.
Before you begin
- You are on, or have just finished, a shift with the participant.
- Your organisation may require a note before you can clock out.
Start the note
- On the shift screen, tap Write note. Or tap Write next to the note item when clocking out, or Write a progress note on the participant’s profile.
- The Progress note screen opens, linked to the shift. If the shift has one participant they are already chosen; otherwise tap the participant’s name.
Add goals and mood
- Under Goals this note supports, tap each of the participant’s goals the shift worked towards.
- Under How was (the participant’s name) today?, choose Great, Good, Okay, Low or Upset. A mood is required.

Write what happened
- Tap a prompt to start a paragraph: What we did, Goal progress, How they felt, Choice and control or For the next shift.
- Write in plain words, in the participant’s voice where you can. You need at least 10 words; the counter turns green when you have enough.
- Tap Check my note for a quick review of the wording.
- Add photos under Photos if they help, for example a meal or an activity.
If your organisation has Axis AI switched on, you can tap Draft with Axis AI, type dot points and use the draft. You are still the author and must check it before you submit.
Flag and submit
- Switch on Possible incident if something happened; the app helps you write an incident report next.
- Switch on Health concern to notify the RN on call.
- Tap Submit note. The button tells you what is missing if it isn’t ready, for example Choose a mood or Write at least 10 words.
- A message confirms the note was submitted and how many goals it is linked to.
Tips
- Notes can’t be edited once submitted. If your team leader returns a note for changes, you get a notification.

If something goes wrong
“A note for this shift already exists”
A note has already been submitted for this shift and participant. Ask your team leader if you need to add to it.
Draft with Axis AI says it isn’t switched on
Your organisation doesn’t have Axis AI. Write the note yourself.
Submit note stays grey
Check a participant and a mood are chosen, the note has at least 10 words, and any photos have finished uploading (or remove a failed one).
