Support worker app · WA-005

Reporting an incident from the app

Report an incident from your phone as soon as everyone is safe: what happened, how serious it is, who was involved, injuries and what you did, with photos if useful.

Before you begin

  • Everyone is safe. If anyone is in danger, call 000 first.
  • You are signed in to the support worker app.

Open the incident report

  1. On the shift screen, tap Incident. This links the report to the shift and its participants.
  2. Or tap the round button on any main screen and choose Report an incident, or tap the incident icon on a participant’s profile.
  3. When clocking out, choosing I reported an incident also opens the form.

Say what happened

  1. Under What happened, tap the incident type.
  2. Fill In one line with a short summary.
  3. Fill What happened, in order: what you saw and heard, and what you did.
  4. Under How serious, choose Low, Medium, High or Critical. Some types set this for you.

Add when, where and who

  1. Under When and where, check the time and tap Change if it happened earlier.
  2. Choose House, Community, Vehicle, Day program or Other, and add Where exactly (optional).
  3. Under Who was involved, tap each participant involved. Add Others who saw it (optional).

Record injuries and immediate actions, then submit

  1. Describe any injury under Injuries and first aid (leave it empty if none).
  2. Under What you did straight away, tap everything that applies, such as Made everyone safe, Gave first aid, Called 000 or Followed the behaviour support plan.
  3. Switch on Police were involved if they were, and add Photos if they help.
  4. Tap Submit report. Your team leader is told straight away. If it may be reportable to the NDIS Commission, the message says so and when the notice is due; your team leader handles the notice.

Tips

  • Your recent reports and their status are listed at the bottom of the form under Your recent reports.
  • Report as soon as you can, while details are fresh. You don’t need to decide whether it is reportable; your team leader does.

If something goes wrong

Submit report stays grey

Choose an incident type and fill in both In one line and What happened, in order. Wait for photos to finish uploading, or remove a failed one.

No signal when reporting

If your organisation has offline capture on, the report is saved on the phone and sent when you reconnect. Otherwise call your team leader or the on-call coordinator and report it when you have signal.

The participant isn’t in the Who was involved list

Open the form from the shift so its participants are listed, or check you are rostered with them.
