Recording observations: temperature, blood pressure and weight

Record vital signs and weight against a participant’s safe ranges, and see what happens when a reading is out of range.

Before you begin

  • Your role can record health entries.
  • For out-of-range alerts, a nurse has set safe ranges or the organisation defaults apply.

Record an observation

  1. Go to Care → Health and high-intensity.
  2. Choose the participant in the Participant picker at the top.
  3. Select Record observation. The Record an observation panel opens.
  4. Choose the Observation: Blood pressure, Pulse, Temperature, Oxygen saturation, Blood glucose (BGL), Weight or Respiration.
  5. Enter the Value (for blood pressure, enter Systolic and Diastolic). The units shown are mmHg, bpm, °C, %, mmol/L, kg and breaths/min.
  6. Add a Note if useful, then select Save observation.

What happens with out-of-range values

  1. As you type, the panel shows the participant’s safe range. A value outside it shows a red Out of range callout.
  2. Saving an out-of-range or critical value alerts the escalation list and the message says so. Follow the escalation steps in the participant’s health plan.
  3. The alert appears in the Alerts and oversight tab for a nurse or team leader to acknowledge and review.

Review trends

  1. Open the Observations tab.
  2. The charts show the last 14 days of systolic blood pressure, blood glucose or weight, with the safe range shaded.
  3. Use Recent observations below, filtered by Observation or Range (Out of range / In range), to see individual readings. Use Export to download them.

Set safe ranges

  1. Open the Thresholds tab. Only nurses can edit it.
  2. Enter low and high for each measure, or pick a new measure from Add a measure….
  3. Select Save thresholds. New entries are checked against these ranges; measures without a range use the organisation defaults.

Tips

  • Observations are recorded on the web Health page. The mobile app shows medication and high-intensity supports on the person’s Health tab but has no observation form.

If something goes wrong

Record observation is missing.

Choose a person in the picker first. The button also needs permission to record health entries.

Save observation is greyed out.

Enter a number in Value. For blood pressure, both systolic and diastolic are needed.

Thresholds can’t be saved: “Low must be below high”.

Fix the measure named in the message so low is below high.
