Upload an interim or comprehensive behaviour support plan, set its review date and record lodgement with the NDIS Commission. Uploading again for the same participant adds a new version and keeps the old one.
Before you begin
- You need the Manage behaviour support plans permission (Operations manager, Behaviour support practitioner, Quality and compliance and Administrator roles have it by default).
- The participant already has a record in CompanionAxis.
- You have the plan as a PDF, DOC or DOCX file.
Upload a new plan
- Go to Care → Behaviour support.
- Select Upload a plan at the top right. The Upload a behaviour support plan panel opens on the right.
- Choose the Participant.
- Choose the Plan type: Comprehensive (the default) or Interim.
- Choose the Practitioner. The list shows employed practitioners and external practitioners with active access. If the author is not listed, choose Someone else (enter a name) and type their name in Practitioner name.
- Check Plan start (defaults to today) and Review due (defaults to one year later).
- Add a short note in What changed in this version, then attach the file under Plan document.
- Select Upload plan. You see Behaviour support plan uploaded and the plan opens in the list.
While anything is missing, a yellow box at the bottom of the panel lists it and Upload plan stays greyed out.
Record the restrictive practices and acknowledgement settings
These options sit at the bottom of the same panel.
- Under Regulated restrictive practices in this plan, tick each type the plan contains: Chemical, Environmental, Mechanical, Physical or Seclusion.
- For each ticked type, describe the practice in the box that appears (for example what the restraint is and when it may be used).
- Leave Ask every rostered worker to read and acknowledge the new version ticked so workers are asked to read it. See Making sure workers read the plan before a shift.
- Tick Already lodged with the NDIS Commission only if the plan has already been lodged.
Each practice is added to the register with No authorisation until someone records one. See Recording authorised restrictive practices for a participant.
Upload a new version of an existing plan
- Select Upload a plan and choose the same participant and the same Plan type as the current plan.
- Check the line under the panel title. It says the participant has a current plan and that uploading adds a new version and marks the current one superseded.
- Update Review due if the review date has changed. Plan start is locked for a new version.
- Describe the changes in What changed in this version and attach the new file.
- Select Upload plan. You see Version 2 published. The previous version is marked superseded and workers are asked to acknowledge. (with the right version number).
Check review dates and lodgement
- On Care → Behaviour support, read the tiles at the top: Current plans, Review due in 30 days, Not lodged with the Commission and Acknowledgements outstanding.
- Select a plan in the Plans list. The detail card shows Practitioner, Plan type, Version, Started, Review due with a days-left badge, and Status.
- If the plan shows Not lodged, lodge it with the NDIS Commission, then select Mark lodged on the plan card. The card then shows Lodged with Commission.
- Open Version history to see every version with its upload date, who uploaded it, the change note and the acknowledgement count.
Tips
- An interim plan shows a reminder that a comprehensive plan must be in place within 6 months of the interim plan starting, with the date worked out for you.
- A plan moves to Review due automatically 30 days before its review date, and the practitioner and operations managers get a Behaviour support plan review due notification.
- You can open the participant’s record from the plan card name, or filter the page to one participant from the record’s Behaviour tab.
If something goes wrong
A comprehensive plan must be reviewed at least every 12 months.
The review date is more than 12 months after the start. Bring Review due forward.
An interim plan must be replaced by a comprehensive plan within 6 months.
For an interim plan, set Review due no more than 6 months after Plan start.
Review date must be after the start date.
Check both dates. The review date must be later than the start date.
The What workers see on shift card says Not yet entered for triggers and strategies.
The upload panel does not have fields for strategies. Strategies carried forward from an earlier version still show. Ask your administrator how strategies are entered for your organisation.
