Building a care plan and linking it to NDIS goals

Write a person-centred care plan, add goals linked to the participant’s NDIS plan goals, and take a version through approval, signature and publishing.

Before you begin

  • Edit care plans to draft, Approve care plans to approve or publish, Collect care plan signatures to sign, and Manage goals to add goals.
  • The participant already has a care plan record (see the note below).

Open the care plan

  1. Go to People → Care plans and goals and select the participant, or open the participant and the Goals and care plan tab, then Open care plan.
  2. The page shows the version number, review date and signature status, with the tabs Plan, Goals and outcomes, Reviews and Versions (and Behaviour support when a behaviour support plan is linked).

Write the plan sections

The standard template has About me, How I communicate, People important to me, Daily routine, Risks and how we manage them, and Mealtime and health.

  1. On the Plan tab, select Edit on a section.
  2. Write the section in the participant’s words where you can. The grey prompt shows what belongs there.
  3. Select Save as new version (first edit) or Save to draft (later edits). A draft opens and the published version stays in use until the draft is published.

Add goals linked to the NDIS plan

  1. Open the Goals and outcomes tab and select Add goal.
  2. Enter the Goal in [name]’s words, for example “Make my own breakfast on weekdays”.
  3. Choose the Linked NDIS plan goal from the goals on the current NDIS plan, or leave it Not linked.
  4. Choose a Domain, a Review on date and the Worker responsible.
  5. List Strategies (one per line) and select Add goal. Workers see active goals when writing a shift note.

Approve, sign and publish the version

  1. In the blue bar above the tabs, select Submit.
  2. An approver selects Approve or send back, adds a Comment (required to send back) and chooses Approve or Send back.
  3. Select Sign, choose Who is signing (the participant, a nominee or a guardian), tick the agreement statement, type the full name and select Sign and save, or select Send a signing link.
  4. Select Publish. To publish without a signature, enter the Reason if publishing unsigned first. Rostered workers and the house team are told the plan changed.

Give the participant an easy-read copy

  1. Select Easy-read version, then Edit, and write one idea per paragraph.
  2. Select Publish, then Share to send it by email or SMS, record a printed copy, or select Print.

Tips

  • By default new versions need approval and the participant’s signature, and the approver should not be the author.
  • Every signed version is kept on the Versions tab with a PDF download.

If something goes wrong

The record shows No care plan and the Create care plan button only opens the care plans list

There is no screen to start a care plan yet. Ask your CompanionAxis administrator to create it; once it exists you can edit it as above.

Linked NDIS plan goal shows “No goals recorded on the current NDIS plan.”

The goals come from the participant’s active NDIS plan. Add the goal unlinked for now and link it when the plan goals are recorded.

Send back is greyed out

Enter a comment first; it is required when sending a version back.
