Deciding whether an incident is reportable

The incident form asks the reportable incident questions and works out the category and the NDIS Commission deadline from your answers. You can confirm or change the classification on the incident afterwards.

Before you begin

  • To classify an incident after it is logged you need the Classify reportable incidents permission (Team leader, Operations manager, Quality and compliance and Administrator by default).
  • The house or branch must have a state set, so business days and public holidays can be counted.

Answer the reportable check while logging

  1. On the Reportable check step of Log an incident, answer each question with No, Yes or Unsure. The questions cover death, serious injury, abuse or neglect, unlawful sexual or physical contact or assault, sexual misconduct, and unauthorised restrictive practices.
  2. If you answer Yes or Unsure to the restrictive practice question, also answer Did the restrictive practice cause harm (injury or distress needing treatment)?. The question lists the practices authorised for the participant.
  3. Set When did a manager first become aware?. The notification timeline runs from this time.
  4. Read the Result: a red box says Reportable with the category and the time the NDIS Commission must be notified by (and the 5-day form due time where it applies); a green box says Not reportable.

Unsure counts as yes. If you are not sure, choose Unsure: notifying early is always safer.

How the deadline is worked out

  1. Death, serious injury, abuse or neglect, unlawful contact or assault and sexual misconduct must be notified within 24 hours of becoming aware.
  2. After a 24-hour notification, a 5-day form is due by the end of the 5th business day.
  3. An unauthorised restrictive practice with no harm must be notified within 5 business days; with harm, within 24 hours.
  4. Business days skip weekends and the public holidays of the house’s state. Any holidays skipped are listed in the result.

Classify or reclassify on the incident

  1. Open the incident from Quality → Incidents.
  2. On the Overview tab, find the Classification card.
  3. If it is reportable, choose the category from Choose a category…, add a Reason (recorded in the audit log) and select Mark reportable. You see Classified as reportable. The Commission timer has started.
  4. If it is not classified yet and is not reportable, select Not reportable with a reason.
  5. With the Axis AI add-on, select Suggest with Axis AI, then Approve or Reject the suggestion.

Tips

  • If Treatment needed is ambulance, emergency department or admitted, the form warns that this usually means a serious injury, which is reportable within 24 hours.
  • Abuse or neglect, sexual misconduct, unlawful contact and unauthorised restrictive practice incidents are restricted: only people with access to restricted incidents can read the details.
  • If new information comes to light later, classify it then. The timeline starts from when you became aware.

If something goes wrong

Answer every question. Choose “Unsure” if you do not know yet.

Every question needs an answer before you can continue.

The result says it is worked out when you submit.

No house or branch was chosen. Choose one on step 1 to see the deadline now.

State unknown: Set the state of the house or its region so business days can be counted.

Ask an administrator to set the state on the house or its region.

There is no Classification form on the incident.

It is already reportable, or you do not have Classify reportable incidents.
