Use the Incident trends page to see incidents, reportable rates, timeliness and complaints over 30, 60 or 90 days, and export it for your board or quality committee.
Before you begin
- You need the View incident analytics permission. Exporting needs Export incidents.
- The By participant chart needs access to participant records; the complaints card needs access to the complaints register.
Open the trends page
- Go to Insights → Incident trends, or select Trends on Quality → Incidents.
- Choose the period: 30 days, 60 days or 90 days.
- Read the tiles: Incidents (with near misses and the rate per 1,000 support hours), Reportable, Notified on time and Median days to close (with overdue corrective actions).
Read the charts
- Incidents by week splits reportable incidents, other incidents and near misses.
- Severity shows critical, high, medium and low, with a second ring for reportable, not reportable and not classified.
- By type, By house and By participant show where incidents cluster. Select a name to open it.
- Time of day shows when incidents happen, to plan staffing and handovers.
- Complaints and feedback shows feedback by kind and the share acknowledged on time and resolved within timeframe.
Export for the board
- Select Export for board. An Excel file downloads when ready and you see Incident export downloaded.
- For a list of incidents, go to Quality → Incidents, filter the register and select Export.
- For restrictive practice figures, use the Monthly reporting tab on Care → Restrictive practices (see Preparing restrictive practice use reports for the NDIS Commission).
Tips
- Every figure uses the same definitions as the incident register, so numbers match what staff see day to day.
- Watch Notified on time and Corrective actions overdue closely; auditors ask about both.
