Case notes for coordinators and allied health

Record calls, meetings and visits about a participant as a case note on the same timeline as shift notes.

Before you begin

  • Your role can create case notes.
  • The participant exists in CompanionAxis.

Write a case note

  1. Go to Care → Progress notes and select Write case note. The Write a case note panel opens.
  2. Choose the Participant.
  3. Choose the Contact: Phone call, Meeting, Email, Home visit, SMS or Other.
  4. Under What was discussed, record who you spoke with, what was discussed and any concerns raised. At least 20 words are needed.
  5. Under Decisions and actions, record what was agreed and who will do what. This field is required.
  6. Tick any Goals discussed (active goals from the care plan).
  7. Select Save case note. A message confirms it was saved to the participant’s timeline.

From the participant record

  1. Open People → All participants and the participant.
  2. Open the Notes tab and select Add note. The case note panel opens with the participant already chosen.

What happens next

  1. Case notes lock as soon as they are saved, by default, so they do not go to the review queue.
  2. They show on the participant’s Notes tab and in All notes with the type Case note.
  3. To correct a saved case note, open it and use Amend a locked note.

Tips

  • Use case notes for coordination and clinical contact (calls, meetings, emails). Shift notes written by support workers come from the app.
  • Write so a colleague picking up the file tomorrow knows what happens next.

If something goes wrong

Save case note stays greyed out.

Choose a participant, write at least 20 words under What was discussed and fill in Decisions and actions.

Write case note is not on the page.

Your role needs permission to create case notes (coordinators, team leaders and nurses have it by default).

No goals listed.

The participant has no active goals, or your role cannot view goals. Goals are added in their care plan.
