Note templates by support type

Which note template each shift uses, what the standard templates ask for, and how to change the required fields.

Before you begin

  • Your role can view note templates (all care roles can). Changing them needs the configure permission.

How a template is chosen

  1. Every note gets a template from its service type when it is created.
  2. The service type comes from the shift: sleepover, active night and therapy shifts use their own templates; community access and group shifts use Community access; other shifts at a house use SIL shift; anything else uses In-home support. Case notes use Coordinator case note.
  3. The standard set installed for every organisation is SIL shift, Community access, In-home support, Sleepover, Active night, Therapy session and Coordinator case note.

See what each template asks for

  1. Go to Care → Progress notes and open the Templates tab.
  2. Each card shows the template name, the service types it covers, its version and publish date.
  3. Each field shows its label, its kind (for example mood, goals, photos, text) and whether it is Required. The footer shows the minimum length and the number of required fields.

Change which fields are required

  1. Switch the Required toggle on or off for the fields you want to change.
  2. Select Publish on that card. A message confirms the template was published and that new notes use this version.
  3. Notes already started keep the version they started with.

Only people with permission to configure templates (operations managers and quality and compliance by default) can change the toggles; everyone else sees them read-only.

What workers see

  1. In the support worker app, the mood picker, goal chips and photo strip match the standard template fields. Mood is required in every standard template.
  2. On the web note page, Template checks shows which template fields were completed, with required ones marked.

Tips

  • Keep required fields few; workers have to complete every one before they can submit.
  • The standard templates also carry writing prompts (for example “What did they do today?” and “Anything the next shift should know?”).

If something goes wrong

The Publish button is greyed out.

Publish only turns on after you change a toggle on that card.

The toggles do nothing when clicked.

Your role can view templates but not configure them. Ask an operations manager or quality and compliance.

A worker says a field is required but it is off in the template.

Their note started on an older version. Versions are fixed per note; new notes use the latest one.
