Bowel, seizure and fluid charts

Record bowel, seizure and fluid chart entries for a participant and read the chart to spot problems early.

Before you begin

  • Your role can record health entries.
  • The chart has been set up for the participant.

Add a chart entry

  1. Go to Care → Health and high-intensity and choose the participant in the Participant picker.
  2. Select Chart entry. The Add a chart entry panel opens.
  3. Choose the Chart. Only charts active for this participant are listed (for example Bowel, Seizure and Fluid).
  4. Complete the fields for that chart, add Detail if useful, and select Save entry.
  5. If the entry breaks a rule (for example days without a bowel motion or a long seizure), the message says an alert was raised.

What each chart records

  1. Bowel: Result (movement or no movement) and Bristol type (1–7).
  2. Fluid: In (mL), Out (mL) and Drink. The chart tab shows a daily total against the target.
  3. Food: Meal, Portion eaten (%) and Texture.
  4. Seizure: Seizure type, Duration (seconds), Recovery, Rescue medication given, and a required description of what happened. Long seizures raise a critical alert.
  5. Sleep: State (asleep, awake, restless, out of bed).
  6. Repositioning: Position and Skin check.

Read the chart

  1. Open the Charts tab and pick the chart from the switcher.
  2. A banner says whether the chart is up to date, due or overdue, with the last entry time.
  3. The bowel chart shows the last 14 days as coloured tiles; the fluid chart shows daily totals. The entries table lists the last 14 days; filter Alerts only to see entries that raised alerts.

Overdue charts

  1. Charts with a due time (for example fluid, bowel and repositioning) show in Charts overdue at the top and in Charts due or overdue on the Alerts and oversight tab.
  2. Select a chart in that list to jump straight to it.

Tips

  • Charts are switched on per person in their health plan; the panel only lists active ones.

If something goes wrong

“This participant has no active charts. Charts are set up in the health plan.”

No chart has been set up for this participant yet. Ask the nurse or clinical lead to set it up.

Save entry is greyed out on a seizure entry.

Seizure entries need the description of what happened, rescue medication and recovery.

A chart you expect is missing from the switcher.

Only active charts show. Check with the nurse that it was set up for this person.
